Every decision keeps its own receipt.
The trail is append-only: entries are added, never edited away. For any change you can read the whole sequence — which agent proposed it, what validation returned, what the independent reviewer objected to, which policy rule applied, who approved it, and which release carried it into production. Agents write to the trail on the same terms as people, so an autonomous change is no harder to account for than a hand-made one. Audit integrity is verified, so a missing entry shows up as a break rather than a quiet gap.
Reconstruct the decision, not just the diff
GitHub holds the commit, the pull request and the review; the trail holds why they exist — the proposal, the objection, the rule and the signature. Reading it back does not depend on anyone remembering the conversation.
Refusals and escalations are entries too
A change the policy engine refused, an action that fell outside an agent's scope and became an escalation, and a widened scope with the approval behind it all land in the same sequence. What was stopped is as readable afterwards as what shipped.
Answers the questions reviewers ask
Who approved this change to a customer-data field, what reviewed it, and which policy was in force — each resolves to one entry in the trail. The evidence is produced as the work happens, not assembled the week before an audit.

The rest of Governance
Deterministic Validation
Structure is checked first. A change that does not fit the Blueprint is rejected before any model reads it.
Policy Engine
Rules you can read out loud: deleting a published capability is blocked, reshaping a high-risk entity needs a named approval.
Risk & Approval
Each change carries a risk level, and the level decides: apply automatically, wait for a person, or refuse.
Data Classification
Mark a field sensitive once in the Blueprint, and that marking follows it into generated code and policy.
